The labrum is the attachment site for the shoulder ligaments and supports the ball-and-socket joint and the rotator cuff tendons and muscles, and it contributes to shoulder stability. Symptoms of a torn labrum can include pain and ache in the affected area, weakness or instability in the joint, and a popping sensation. The two most common types of labral tears are the SLAP tear tear and Bankart tear. Both types of tears are usually accompanied by aching pain and difficulty performing normal shoulder movements.
SLAP stands for “superior labrum from anterior to posterior.” This type of tear occurs at the front of the upper arm where the biceps tendon connects to the shoulder. Patients with SLAP tears may experience pain at the front of the shoulder near the biceps tendon.
Bankart tears occur in younger patients who have dislocated their shoulders. When the shoulder joint ball slips out of the socket, the joint capsule can pull on the lower portion of the labrum and tear it. A dislocation where the head of the humerus shifts toward the front of the body, it leads to what is called “anterior instability.” When the ball slips toward the back of the body, it leads to “posterior instability.”

Diagnosing a labrum tear involves a physical examination and most likely an MRI, CT scan and/or arthroscopy of the shoulder. Treatment varies depending on type, severity and location of the labrum tear.
If there is a large tear of the labrum, the torn part should either be cut out and trimmed, or it should be repaired. Which treatment is used depends upon where the tear is located and how big it is.
